At Fearnhead Cross Medical Centre, we understand that your health is influenced by much more than medical conditions alone. Worries about finances, housing, loneliness, caring responsibilities or emotional wellbeing can all affect your health.

That’s where Social Prescribing can help.

What is Social Prescribing?

A Social Prescriber (also known as a Social Prescribing Link Worker) provides personalised, practical support by connecting you with local services, groups and organisations that can improve your health and wellbeing. They take time to understand what matters most to you and help you find the right support for your individual needs.

A Social Prescriber may be able to help with:

• Loneliness and social isolation
• Low mood, stress or anxiety
• Housing or financial concerns
• Benefits and debt advice
• Bereavement support
• Carer support
• Healthy lifestyle changes
• Exercise and wellbeing activities
• Volunteering and community groups
• Living well with a long-term health condition

How Do I Access the Service?

If you’re registered with our practice, you can find out more about our Social Prescribing service and the support available by visiting:

East Warrington PCN Social Prescribing Portal

You can also speak to one of our reception team, GP, nurse or another healthcare professional, who can refer you directly to our Social Prescriber if you would benefit from extra support. The service is available to patients across the East Warrington Primary Care Network and referrals are made through the practice.
