How to register?

Online Registration:

The quickest and easiest way to register is online.

Please click below to complete our online registration form. Once submitted, your application will be sent securely to the practice for processing.

Once your registration has been completed, we will send you a text message inviting you to book a New Patient Health Check with one of our Healthcare Assistants. This appointment allows us to collect some basic health information while we wait to receive your medical records from your previous GP practice.

New Patient Registration - Westbourne Medical Group

Before registering, please check that your address falls within our catchment area by entering your postcode using the search (magnifying glass) in the map below.

 

In person registration:

You can also register by visiting either surgery and completing a registration form.

If possible, please bring:

  • One form of photographic identification (such as a passport or driving licence).
  • One proof of address (such as a recent utility bill or bank statement).

If you are unable to provide identification, please speak to a member of our Reception Team. Lack of identification will not prevent you from registering with the practice.

You will also be asked to complete a short New Patient Questionnaire and book a New Patient Health Check with one of our Healthcare Assistants.

 

Families

As a family practice, we encourage all members of the same household to register with us where possible. This helps us build a better understanding of your family’s healthcare needs and provide continuity of care.

Choosing a GP

All registered patients are allocated a named GP, who has overall responsibility for coordinating their care.

You are welcome to request appointments with any of our GPs, and having a named GP does not mean you can only see that doctor. While we will always try to accommodate your preference, it may not always be possible at short notice.

If you would like to change your named GP, please contact the surgery and a member of our team will be happy to help.
