New Patients – register online

We can only accept registration requests from patients who live within our practice boundary, you can search for your postcode by clicking the magnifier in the top right corner of the map below, this will indicate whether you live within the boundary or not.

WA1 & WA2 are in the Practice Area

WA3 & WA5, some of these postcodes are in the Practice Area and some are out – please check using the map above before filling out your form.

WA4 & WA6 onwards are all out of our Practice Area.

 

Online Registration:

Please click this link , you will be directed to a web page and asked to fill out a form which once completed will be sent to us at the Surgery.

When you are registered, we will send you a text message with a link for you to book an appointment with a Health Care Assistant for a registration check. This appointment enables us to have some medical history about you until we receive your medical records from your previous GP.

In person registration:

You can go to either surgery and complete a patient registration form. You will need to bring 2 forms of I.D. to register, one of which must be photo I.D. and the other proof of address. If you are struggling to provide I.D. please speak to us, it does not mean you will be declined registration.

As we are family Doctors, we prefer that all members of the family register with the Practice, as this provides the Surgery with a complete history of the patients and family circumstances. Patients have the right to express a preference of Practitioner; however, it may not always be possible to see their preferred Doctor at short notice.

You will also be asked to complete a short New Patient questionnaire and book a New Patient Registration Check with a Health Care Assistant. This enables us to have some medical history about you until we receive your medical records from your previous GP.

 

 

Named allocated GP – All patients that register will have a named GP allocated to them that will be accountable for their care. It doesn’t mean you only have to see that GP and patients can choose to see a GP of their choice. If you wish to change the GP you have been allocated, please speak to a member of the team
